Sorting out fertility medication coverage can feel overwhelming, but you don't have to do it alone.
This page explains how pharmacy benefits work, why speciality pharmacies are important, medication savings programs you may qualify for, and other practical ways to lower out-of-pocket costs.
Schedule Your Consult Get Savings ChecklistMany insurance plans cover procedures and prescriptions under different benefit "buckets."
Fertility medications (like stimulation injections, trigger shots, and progesterone) are often billed under pharmacy benefits with their own copays/coinsurance, tiers, and quantity limits.
Some insurance plans require prior authorization for certain drugs, and many specify a preferred specialty pharmacy.
After your first consultation with an Illume physician, we’ll confirm your plan's coverage rules and handle any required authorizations so prescriptions always line up with your expected treatment timeline.
Schedule Your Consult Get Savings ChecklistLearn the biggest factors that change what you pay and simple ways to spot savings.
Some medicines are brand-only biologics with no generic option, while others have lower-cost equivalents your plan may cover.
Pen injector vs. vial, prefilled vs. multi-dose, and form affect price. Mismatched vial sizes can leave you paying for unused medicine.
Insurers sort medicines into tiers (generic, preferred or non-preferred brand, specialty). Each has a different copay or % you must pay.
Many plans require a specific specialty pharmacy; their shipping, fill options, and negotiated rates can raise or lower what you pay.
Requirements like prior authorization, quantity limits, or "step therapy" can change which medicine you get and what it costs.
A quick overview of the most frequently prescribed fertility medicines (both brand names and lower-cost alternatives, where they exist).
We’ll always recommend the most affordable, clinically appropriate option your plan allows. Your exact prescriptions will be tailored to your treatment protocol.
Note: FSH injections (e.g., Gonal-f®, Follistim AQ®) are biologic drugs with no true generics, so they’re only sold as brands - meaning they typically cost more.
Other medications (like ganirelix or micronized progesterone) often have generic versions.
See Typical IVF ProtocolGonal-f® (follitropin alfa), Follistim AQ® (follitropin beta), and Menopur® (menotropins/hMG).
Cetrotide® (cetrorelix) and Ganirelix acetate (generic).
Ovidrel®, hCG (Pregnyl®, Novarel®), and Leuprolide (Lupron).
Progesterone in oil (IM), Endometrin® (vaginal insert), Crinone® (vaginal gel), and micronized progesterone capsules (generic).
Letrozole (Femara®), clomiphene citrate (Clomid®), leuprolide (generic), and cabergoline (generic).
Many insurers (and fertility benefit platforms) require that your prescriptions be filled through an in-network specialty pharmacy.
Here's why: These pharmacies offer fertility-savvy support, cold-chain shipping (to keep medications safe and effective), on-call pharmacists to answer questions, and options like split fills (where possible) to reduce waste if doses change mid-cycle.
We’ll route your prescriptions to the approved pharmacy, coordinate prior authorizations, and recommend possible ways to save. If your plan switches preferred pharmacies mid-cycle, we’ll assist with transfers so your treatment timeline doesn't get disrupted.
Schedule Your Consult Get Savings ChecklistProtect your fertility medications (and your wallet) with these practical but important steps.
If a pen or vial arrives cracked, cloudy, or warm, call the pharmacy and do not use it.
Verify medication expiration dates before you accept delivery or pay at pickup.
Follow the insert’s storage rules for refrigeration, light sensitivity, and temperature range.
Six practical tactics to decrease out-of-pocket costs without affecting your treatment.
Check the cost per IU (for example, compare pen vs. vial form of the medication) so you’re only paying for medicine you’ll actually use.
If you’re close to your deductible or out-of-pocket max for the year, refilling after you reach it can make the same medications cost far less.
Pay with pre-tax dollars to lower your effective cost (often 20–30% depending on your tax bracket). Keep records in case of an audit.

Clear answers to common pharmacy and savings questions.
Often yes—many plans require that you use an in-network specialty pharmacy for fertility medications.
We’ll confirm which one your plan allows, route prescriptions there, handle prior authorizations, and point you towards copay cards or manufacturer savings.
If your plan switches pharmacies or you prefer a different in-network option, we’ll help transfer prescriptions so your treatment plan stays on track.
Frequently used fertility medications you may encounter include:
Most insurance plans bill these under pharmacy benefits that involve specific tiers, copays/coinsurance, and sometimes, prior authorization.
Some medications may fall under medical benefits instead. Your Illume Financial Coordinator will help verify these details and estimate your share for each.
The answer to this will depend on your plan. Your Financial Coordinator can help confirm whether medication costs will apply to medical or pharmacy accumulators.
Ask your Illume nurse and Financial Coordinator whether it's possible (and allowed by your plan) to purchase smaller quantities of medication more often. This can lower upfront costs and avoid medication waste.
However, this also requires extra careful attention to ensure you don't run out of needed medications during your cycle. Always discuss the pros and cons with your Care Team.
If you end up with leftover medication, most fertility clinics (including Illume Fertility) will accept donations of unopened, unexpired medications, then redistribute them to patients in need.
Contact your Care Team right away. We’ll follow up with your insurer and pharmacy, help arrange expedited shipping if needed, and discuss safe alternatives with your physician to keep your cycle on track.
Often, but not always! Some IVF meds don’t have true generics (they’re what's called biologics). We will always prescribe the most affordable option anytime there is a lower-cost alternative that is both clinically appropriate and effective.

Early in the plan year, you are still paying toward your deductible. Later, once you’ve met your deductible or out-of-pocket maximum, covered medications can cost much less.
Some insurers may also change preferred brands mid-year; your Care Team will provide guidance and adjust your protocol quickly if that happens.
